Ultimate Guide to Realistic Health Mods in The Sims 4 by Adeepindigo
The Sims 4 has never been recognized for its intricate or immersive health simulation mechanics. Traditionally, sims are either perfectly healthy or suddenly dealing with a generic cold, which can be instantly cured by a universal medicine. Many Sims 4 players often find themselves wondering: why not increase the life simulation aspect by installing realistic health mods for The Sims 4?
Adeepindigo stands out as one of the most renowned Sims 4 mod creators in the area of advanced health systems. Their Sims 4 mods transform basic sim health into a deeply complex, realistic, and highly engaging experience. With these mods, players can add everything from detailed dental health and the need for contact lenses, to mental health conditions, neurodivergence, and in-game therapy options. We've selected five essential health mods for The Sims 4 that will completely change how you play and perceive the game.
Healthcare Redux: A Complete Sims 4 Healthcare System
The Healthcare Redux mod for Sims 4 is the most extensive and significant health overhaul available for the game. Instead of just providing new minor illnesses, this Sims 4 mod introduces a comprehensive healthcare system. It covers everything from natural immunity and chronic diseases to doctor visits, medical insurance, and even realistic pregnancy and postpartum complications.
It's important to mention that Healthcare Redux is a modular Sims 4 mod, meaning you are not required to install every feature. You can customize your gameplay experience by choosing only the health modules that interest you most. Just remember to keep the core mod files, labeled as CORE, intact for proper functionality.
Key Features of Healthcare Redux
Natural immunity levels. Each sim is now assigned a unique immunity rating – weak, normal, or strong – which determines how often and how severely they contract illnesses.
New diseases in Sims 4. In addition to common ailments like colds, bronchitis, gastroenteritis, and tonsillitis, this mod introduces life-threatening illnesses such as bacterial meningitis and tuberculosis. Without timely Sims 4 medical treatment or vaccinations, sims can face fatal outcomes.
Chronic health conditions. Sims may be diagnosed with anemia, anxiety, asthma, arthritis, eczema, insomnia, migraines, seasonal affective disorder, diabetes, or hypertension. Managing these chronic conditions requires ongoing treatment and adds a new layer of realism to Sims 4 health gameplay.
Allergies. Sims 4 Healthcare Redux enables sims to develop allergies to bee stings, pet dander, or seasonal pollen, adding even more diversity to daily life challenges.
Injuries and emergency situations. Child sims can receive random injuries while athletic sims risk sports-related injuries, some of which may require surgery. The mod also simulates emergency medical events such as postpartum hemorrhage, heart attacks, aneurysms, anaphylaxis, and even car or airplane crashes.
Pregnancy and postpartum mechanics. The Sims 4 pregnancy system is now truly realistic. Features include prenatal doctor visits, complications such as preeclampsia, anemia, or gestational diabetes, as well as postpartum depression and risks like postpartum hemorrhage.
Treatment and prevention options. Sims can purchase both over-the-counter and prescription medications from an online pharmacy, take daily vitamins, receive vaccinations, use inhalers, and utilize various medical remedies.
Doctor appointments. The Sims 4 "Medical Services” section is now accessible via the in-game smartphone, allowing Sims to schedule appointments with general practitioners, allergists, medical specialists, obstetricians/gynecologists, sports medicine clinics, or even acupuncturists.
Hospitalization in Sims 4. Serious health conditions like appendectomies, tonsillectomies, injury repairs, or kidney transplants will now send your sim to the hospital, adding a realistic dynamic to severe cases.
Health insurance plans. Every Sims 4 family receives a health insurance package based on their sims’ jobs. Choose from four different insurance plans (including a universal, free government plan) or opt out and pay all medical bills yourself.
Modular health system. Thanks to a modular setup, players can install only the health modules they want: for illnesses, allergies, chronic diseases, injuries, pregnancy, or emergencies.
How to Install Healthcare Redux
Download the mod directly from the official creator’s website. All other Adeepindigo Sims 4 mods follow the same approach. Just visit the dedicated page and find the download button at either the top or bottom of the section.
Once clicked, a pop-up window will appear suggesting you support the creator on Patreon – skip this step if you’re not interested.
Proceed, and you’ll be taken to a page listing all available Sims 4 Healthcare Redux mod files.
Additionally, the mod author includes recommendations for companion mods that ensure full functionality and instructions for disabling death from diseases if you wish. If you want your sims to get sick without risk of dying, simply place (or do not remove) the ModDeath_GlobalDisabler file from the Optional Addons folder. This folder also contains other files for additional features, which you may keep or discard based on preference.
Healthcare Redux requires the installation of Lumpinou's Mood Pack and General Pie Menus, along with The Sims 4: Get to Work expansion pack. Without these dependencies, the mod will not function properly. Note that Adeepindigo often includes the General Pie Menus file in every Sims 4 mod archive, but only one copy should be present in your Mods folder. We recommend moving this file to a clearly marked standalone folder and deleting any duplicates whenever you install new mods.
Mod Compatibility: Healthcare Redux is incompatible with Private Practice and Slice of Life. If you have both mods installed, only Healthcare Redux will remain active, and Private Practice modules will be disabled. Mixing modules from different Sims 4 health systems is not recommended by the mod creator.
Using Healthcare Redux Without The Get to Work Expansion
Players who do not own The Sims 4: Get to Work expansion can still enjoy the Healthcare Redux mod. A base game version is available, offering slightly fewer modules and features, but providing compatibility for those with only the core Sims 4 game installed.
Dental Care: dental hygiene and dentist visits
Dental Care adds to the game what many simmers have been missing: real consequences for not brushing teeth.
How it works
Dental health. Each sim has a hidden dental health parameter that deteriorates without care. Regular tooth brushing, using mouthwash, and mints help maintain it.
Dental problems. If you don't keep up with hygiene, sims develop cavities, tooth sensitivity, toothaches, may require root canals or implants.
Bad breath. If a sim hasn't brushed their teeth for more than 24 hours, they get bad breath. Others feel uncomfortable.
Dental supplies. Using a computer, you can order toothpaste, mouthwash, mints, and breath freshening sprays.
Dentist visits. Sims can schedule routine cleanings (every 14 days), appointments for problems, and surgical procedures. For children and teens there are braces, for teens and young adults – wisdom tooth removal.
Dental insurance. Working families receive insurance that provides a 40% discount on visits.
Installing the mod
The action algorithm is the same: go to the website, click the same buttons and the mod is yours. The mod requires Lumpinou's Mood Pack. If you've already downloaded it before, then everything's fine and you don't need to download anything again.
Dental Care consists of modules, you can remove serious dental problems such as braces installation, wisdom tooth removal, root canal treatment, and dental implants.
Note that the mod conflicts with any mods that modify the sink_Brush_Teeth and loot_sink_Brush_Teeth files, including faster tooth brushing mods (Faster Teeth Brushing, Brush the Taste Away). In Dental Care itself, tooth brushing is already sped up, so a separate mod isn't needed.
Eye Care: vision, glasses and laser correction
Eye Care allows sims to have vision problems and solve them.
What the mod offers
Hidden vision indicator. Each sim has their vision quality randomly determined – good or bad. With age, everyone's vision deteriorates.
Deterioration from strain. Reading, working at a computer, watching TV, using a phone and tablet accelerate vision deterioration.
Diagnoses. Myopia, hyperopia, cataracts (in adults and elders), glaucoma (in elders), conjunctivitis (in children and teens).
Visits to the optometrist. Every 21 days you can schedule an eye exam. After diagnosis, a sim can get glasses or contact lenses.
Laser correction. Young adult and adult sims with myopia or hyperopia can have surgery that will completely correct their vision.
Vision insurance. As with other mods in the series, working families receive insurance with a discount on visits and surgeries.
New social interactions have been added: complain about poor vision, joke about glasses, brag about excellent vision.
Installing the mod
The mod doesn't require expansions and is compatible with the base game. Downloading happens exactly the same way as the previous times. Then unpack the mod archive and place it in your Mods folder.
The mod has no modules, so you should only have two files in your folder.
Divergent Sims: neurodivergence, mental health and therapy
Divergent Sims is adeepindigo's most extensive mod. It adds a realistic system of neurodivergence, mental health, self-esteem and addictions to the game. This isn't just a set of character traits, but a deep system that affects behavior, skills, social interactions and even treatment.
Main mod mechanics
Self-esteem and neurodivergence. All sims have their self-esteem (low, neutral, high) and neurotype (neurotypical or neurodivergent) randomly determined. This is the foundation on which everything else is built.
Diagnoses by age. Toddlers can have developmental delays (speech, motor, cognitive, social-emotional). Children – learning difficulties (dyslexia, dysgraphia, dyscalculia, dyspraxia). Teens and older – a wide spectrum of conditions.
Neurological conditions. Attention deficit hyperactivity disorder (ADHD), autism spectrum disorders (with three severity levels), giftedness (which is also considered neurodivergence).
Anxiety disorders. Generalized anxiety disorder, obsessive-compulsive disorder (OCD), post-traumatic stress disorder (PTSD).
Mood disorders. Bipolar disorder, clinical depression, seasonal affective disorder.
Personality disorders. Borderline personality disorder, histrionic personality disorder, paranoid personality disorder.
Eating disorders. Anorexia, bulimia, body dysmorphia (affect attitude toward food, mirrors and physical activity).
Addictions. The mod has integration with Basemental – a sim can start abusing substances, using it as a way to cope with difficulties.
Diagnosis and treatment
Psychiatrist. Parents can take a child to a child psychiatrist for developmental assessment. Adult sims can schedule an appointment on their own.
Therapy. Different types of therapy are available: speech therapy, behavioral therapy, play therapy, occupational therapy, psychotherapy, family therapy, support groups.
Medications. Antidepressants, anti-anxiety medications, psychostimulants, antipsychotics, mood stabilizers. All medications have possible side effects.
Self-help. Sims can try to cope independently through exercise, meditation, healthy eating, but the effectiveness is lower than professional help.
Installing the mod
The mod is downloaded from the creator's website and has several modules. As with Healthcare Redux, it's important to keep the core mod modules, and you can install the rest at your discretion.
Note that the mod archive also contains the adeepindigo_base_generalpiemenus_v3 file, which should only be one in the folder. If you already have it, simply delete the file from the mod folder.
The mod requires Lumpinou's Mood Pack and is compatible with the base game. After you install the mod and enter a household, you need to wait 15-20 in-game minutes. A notification should appear with an assessment of your sim's condition, which will mean the mod has fully loaded.
The creator deliberately doesn't block installation with mods that add similar traits (for example, Basemental or YourFalseHope). However, if a sim gets a trait from both there and from Divergent Sims, this can lead to an excessive number of moodlets. The creator recommends choosing one system for each sim.
Personal Sim Hygiene: hygiene in all its forms
Personal Sim Hygiene is a small but important mod that adds details to sims' daily hygiene.
What the mod adds
Dental hygiene. If a sim doesn't brush their teeth for more than 24 hours, they get bad breath, which affects those around them. There's a small chance of getting a cavity, which requires a dentist visit.
Body hygiene. Sims can use lotion for dry skin, deodorant (temporarily slows the decay of the hygiene bar), perfume and body sprays (give confidence or can cause dizziness). Adult sims can apply facial cleanser.
Hair care. At the mirror, you can apply styling product, which gives a temporary positive mood. However, products can build up and require more thorough hair washing.
Hand hygiene. If a sim's hands are dirty, a corresponding moodlet appears. Washing hands removes it.
Installing the mod
The mod doesn't require expansions, but uses animations from Snowy Escape and High School Years (if they're not there, the animations are simply absent).
Core Library mod is also required for proper functionality. The mod consists of modules and, if you have Dental Care installed, the simplehygiene_dental module will conflict with it, better to remove it.
Be careful with base_generalpiemenus_v3, as it's present in this archive too. Delete the file if it's already in another folder.
Installation specifics and possible conflicts
All mods by adeepindigo require following several common rules. If you don't take them into account, the mods may not work or cause errors.
General requirements for all mods
Lumpinou's Mood Pack. This is a required library, without which none of the listed mods will launch. The link to it is on every download page.
General Pie Menus. An interaction menu that's used in many adeepindigo mods. Without it, some options may not appear or cause UI errors. But this file must not have duplicates!
Proper installation. Mods are distributed as archives. Inside is a folder with files. It needs to be placed in the Mods folder, but no deeper than one nesting level (meaning the mod folder can be placed directly in Mods, but not inside another folder). .ts4script files cannot be placed deeper than one sublevel, otherwise scripts won't load.
Cache cleaning. After installing or updating mods, it's recommended to delete the localthumbcache.package and avatarcache.package files from The Sims 4 folder. This helps avoid conflicts with old data.
Keep track of updates. Adeepindigo regularly updates mods, especially after the official Sims 4 patches release. Before installing, check that you have the most recent version.
Together, these five mods transform The Sims 4 into a game where character health is a complex, multifaceted system. Sims can suffer from chronic conditions, need glasses, treat their teeth, visit a therapist and cope with anxiety. This adds depth to characters and makes them more alive.
If you want to make your sims' lives more realistic, it's worth paying attention to this collection. It's not for everyone: some may find it too complex. But for those who love depth and storytelling, these mods will be a real treasure.
